Im most familliar with the Cisco 7960 series, and asterisk PBX...and YES it's possible. I have a customer who utilizes that feature to listen into his business remotely. It takes an entry to the Sftp server that he has to program his phones features, and then a reboot of the individual phone to load the configuration.
In the TSCM mode, one needs to look at the setup and features being enabled on the phone on reboot. I had an instance where I almost thought this was happening to a customer, but it turned out to be a flaky config file which caused the phone to lockup and require a reboot often.

Does anyone know if it is possible to remotely activate the 'hands 
free' function on an IP telephone (using software) to allow listening 
in to room conversations?
What sort of level of capability would be required to carry out this 
threat?
What protocols should the TSCMer use to clarify this threat? What 
countermeasures can be used to defeat this threat?
