Realizing that my chances of recovery and release are improved considerably
by intensive treatment, I request admission to the Compressed Encounter Therapy
Unit on "F" ward. I am ready to meet the conditions of treatment specified

Dear Mr. Graham:
On February the 1st, 1967 Mr. Chapman approved a request for 20 milligrams of Lysergic Acid Dieghylamide (sic) for use at the Mental Health Center, Penetanguishene, Oak Ridge Division. [NAME BLACKED OUT] was the qualified investigator and the purpose of the LSD was for clinical research. Enclosed please find a xeroxed copy of the notice of approval of sale of Schedule H drugs. In September of 1972, we used up the last LSD and are in the process of presenting a paper which summarizes our use of this drug, and when this paper is finished we intend to send it to your Ministry for approval.
It is my intention at the present time to request a further 20 milligrams of LSD to the continued therapeutic use with the patients at the maximum security hospital in Penetang. We would wish to continue to use it as we have in the past, bound by the same conditions or others which might have subsequently emerged. At the present time it is our (sic) understanding that the Connaught Medical Research Laboratories in Willowdale are not dispensing this drug and would also wish to know which drug company has the authority to dispense LSD.
I hope this letter is clear in our request and we would be happy to share any information with you that you might deem helpful. We are also prepared to have a research committee set up. You will also find enclosed the rough draft of our application for purchase of Lysergic Acid Dieghylamide to the director of the Connaught Medical Research Laboratories in Willowdale. It is the intention that I would be the qualified investigator.
