I assume the limitation here is the size of the electronics and the
batteries. The electronics board is likely not possible to make any
smaller by home-made means. The camera module itself may be possible to be
removed from the board and attached on short leads; I did that recently
with a CCD chip from a standard small surveillance camera module, it
worked well on a 5-inch flat ribbon cable with just a minor increase of
susceptibility to electromagnetic interference, which I assume could be
rectified with better shielding. The battery is the part easiest to
modify; by trading size for capacity you can either miniaturize further
for the cost of shorter operating time, or use a bigger cell and then the
flash is the only limit.
Not sure pen is the best form factor. What about a shoe or a briefcase,
handbag, or perhaps a cellphone? A cellphone battery is a fairly big
object that could be made smaller while keeping the functionality, by
replacing the inside with a smaller battery.
Or, if we want to keep the form factor, perhaps a marker. A thick marker
used for writing on whiteboards.
