If both sides have reliable missile defenses then what would be the point in a war? To waste billions of dollars for your missiles to be shot down?

No more MAD. If a war were to occur I suspect it would be a conventional war. Russia may have the support needed to push the US off the Eurasian continent. You must look at the situations in Venezuela, Ukraine( more specifically Crimea), North Korea, Iran, and possibly even Pakistan. God forbid China, or a few European powers, join the war on the side of the Russians. The world is polarizing once again. So if one side pushes the other too far, and there is no threat of MAD, what would stop a new world war? This is all speculation of course.
